Diabetes, COPD, Asthma, Liver Disease, Kidney Disease, Over 65, Heart Disease, Are Pregnant or have a Suppressed Inmmune System

MEDICAL EXAMINATIONS

Appointments for medical examinations in respect of insurance of employment purposes or for HGV/PSV licences should be made by telephoning the Scurlage surgery. Patients should be aware that a fee will be charged for some medical examinations. These are normally done on a Friday afternoon at Scurlage.

CHARGES FOR PRIVATE CERTIFICATES, INSURANCE CLAIM FORMS AND MEDICAL REPORTS

There will be a charge for completing these forms. A list of charges is available upon request.
